About Scissortail Hospice

As a health care provider we realize that you have numerous other agencies to choose from. Scissortail Hospice has been caring for patients since 2006. We are not only Medicare Certified but have earned the status of a "Deemed Hospice" through CHAP (Community Health Accreditation Program). We believe that seeking hospice and palliative care isn’t about giving up hope or hastening death, but rather a way to get the most appropriate care in the last phase of life. Scissortail Hospice provides a compassionate approach to end-of-life care, by enhancing the quality of remaining life and enabling your patients to live as fully and as comfortable as possible.

We have geographically placed complete interdisciplinary teams to care for our patients. Each team consists of a Medical Director, Patient Care Coordinator, Chaplain, Medical Social Worker, Certified Home Health Aide and Volunteers. By having several teams we are able to spend more one on time with our patients and their families, we are able to coordinate care and facilitate professional and personal relationships between the patients, their physicians and our team members.

We are contracted with numerous hospitals and Nursing Homes throughout our coverage area. This is a benefit to our patients, if there is ever a need for inpatient hospice care, respite care, or in the event of permanent residential placement is needed our patients and families are able to utilize these facilities immediately without the wait of negotiating a contract.

During the final days of their lives or crisis situations our patients and their families may possibly need an increase in clinical, emotional, and spiritual care. A few types of symptoms that might require continuous care include uncontrolled pain, unrelenting nausea and vomiting, acute respiratory distress, and terminal restlessness or agitation. With several interdisciplinary teams our agency is never under staffed and work together to provide this continuous level of care.

Each Patient Care Coordinator develops a written care plan that is given to all hospice disciplines for smooth coordination of care. You and your loved one will receive copies of the care plan as well, listing specific duties, visit frequencies, and the contact information for after-hours needs or concerns. Each patient will be seen a minimum of two times weekly by our nurses with other disciplines scheduled based on each individual need. We have a maximum response time of 45 minutes for any after-hours needs that require a visit. In most cases our on-call staff are the same care providers that see your loved one routinely which allow our patients to have continuity of care when they need it most.
